Why Fevers Help (Sometimes)


Dear health-conscious friends,

This week, we are talking.... fever!

When your temperature climbs during an infection, your first instinct may be to bring it down as quickly as possible.

But here’s the interesting part: a fever isn’t always the enemy.

In many infections, fever is actually part of your immune system’s defence strategy. Your body may deliberately turn up the temperature to make life harder for invading microbes — while helping parts of your immune system work more effectively.

So why does fever happen, and when should we leave it alone?

Your Body Has Its Own Thermostat

Deep inside the brain is a region called the hypothalamus, which helps regulate body temperature.

Normally, it keeps your core temperature around 37°C, although normal temperature varies between people and throughout the day.

During an infection, immune cells release chemical messengers called cytokines. These ultimately stimulate the production of prostaglandin E2, which tells the hypothalamus to raise the body's temperature set point.

Suddenly, 37°C feels too cold.

That explains something many people find confusing: why do you shiver when you have a fever?

Your brain has temporarily decided that your current temperature is below where it should be. Blood vessels in the skin constrict to reduce heat loss, and your muscles begin shivering to generate more heat.

You feel freezing — even though your temperature is rising.

Why Would the Body Do This?

Because higher temperatures can provide an advantage against infection.

Some bacteria and viruses reproduce less efficiently at higher temperatures. Raising body temperature may therefore create a less comfortable environment for certain pathogens.

But fever can also affect the immune response itself.

Moderately elevated temperatures may enhance several immune processes, including the activity and movement of white blood cells involved in identifying and attacking infection.

In other words, fever isn't simply something that happens because you're sick.

It can be part of the body's attempt to fight back.

So Should You Always Let a Fever Run?

Not necessarily.

This is where the word “sometimes” matters.

Fever has benefits, but it also has costs. A higher body temperature increases metabolic demand, fluid loss and oxygen consumption. Fever can also leave you exhausted, dehydrated and miserable.

For an otherwise healthy adult with a mild fever from a straightforward viral illness, there may be no medical need to aggressively reduce the temperature simply because the thermometer reads above normal.

Sometimes the goal of paracetamol (acetaminophen) or ibuprofen isn't to “cure” the fever — it's simply to make you more comfortable.

But the situation changes when fever is very high, persistent, accompanied by concerning symptoms, or occurs in someone particularly vulnerable.

When Fever Deserves Medical Attention

Seek medical advice when fever is accompanied by symptoms such as difficulty breathing, confusion, severe headache or neck stiffness, seizures, a concerning rash, severe dehydration or significant deterioration.

Fever in a very young infant requires particular attention. A temperature of 38°C or higher in a baby under 3 months should be assessed promptly by a healthcare professional.

And remember: the number on the thermometer is only part of the picture. How ill someone looks and feels can be just as important.

The Takeaway

We often think of fever as something malfunctioning inside the body.

In reality, it is usually a controlled biological response.

Your immune system detects a threat, communicates with the brain, and temporarily turns up the body's thermostat. That extra heat may make conditions less favourable for some microbes and support aspects of the immune response.

So the next time you're lying under a blanket wondering why your body has decided to transform itself into a radiator, remember:

Sometimes, the heat is part of the fight.
